THIS DEED OF ADOPTION is made on the day of __________ between:
Mr./Mrs. [Adoptive Father & Mother Names] (hereinafter called the “Adoptive Father & Mother”) of the one part,
AND
Mr./Mrs. [Natural Father & Mother Names] (hereinafter called the “Natural Father & Mother”) of the other part.
WHEREAS:
- The Adoptive Father and Mother have [number] [son(s)/daughter(s)] named ____________, aged about ______ years.
- The Adoptive Father and Mother are desirous of adopting a [son/daughter] as per their wishes.
- The Natural Father and Mother have agreed to give their [son/daughter], named ____________________, born on ____________ at ____________, now aged about ____________ (approx.), in adoption to the Adoptive Father and Mother.
The Natural Father and Mother have [number] [son(s)/daughter(s)], excluding the said child being adopted. - [Name of the spouse of Natural Father or Mother], being the spouse of the Natural Father/Mother of the said child, has given her/his consent to the adoption. This consent is evidenced by her/his signature on this Adoption Deed.
- The Adoptive Father is more than ______ years old, and the Adoptive Mother is more than ______ years old than the said child being adopted.
NOW, THIS DEED WITNESSES AS FOLLOWS:
- The Natural Father and Mother, in the presence of their families and friends, at a gathering held at __________ on [Date], have voluntarily given their [son/daughter], named ____________________, in adoption to the Adoptive Father and Mother.
- The Adoptive Father and Mother have formally accepted the said ____________________ as their legally adopted [son/daughter].
